Printed on red cardstock - this form has helped us to have all the
necessary information filled out prior to making calls to the abuse hotline.
It also serves as "easy to spot" documentation in private student files of the reports made.
The front of the form has ample writing space and prompts the
counselor to include all information that will be necessary when
making a hotline call. The other side of the form has a
front/back sketch of a body to easily mark the exact location
and type of marks made on the individual.
